Correct usage:
/me sticks a cigarette in his mouth while reaching for his lighter with the other hand, then lights it up.
wrong:
/me sticks a cigarette in his mouth while lighting it with the lighter he holds in the other hand.
(since obviously, you first need the cig in your mouth to light it, you cannot do so before)

I was doing this mistake as well when a log roll was performed and after backboard was slided underneath, this should be stickied in my opinion!
